Forms for Students Already Registered with ODS
- Letter of Accommodations (LOA) request - All approved students should fill out this form, to request a Letter of Accommodations to be sent to their professors.
- Exam request - Students approved for exam related accommodations such as extended time, reduced distraction testing locations, use of a scribe/reader, etc. should use this form to request our office to proctor your exams.
- Note taking request - Students approved for note taking services such as note takers, use of a digital recorder, or use of a smartpen should use this form to request a note taker in their class.
- Alternate Format request - Students approved for Alternate Format Texts should fill out this form to request our office to convert their course materials.
- Additional parking request - Students who do not have a state issued permanent or temporary (Handicapped Parking) ID or placard and require additional parking should fill out this form to be approved for additional parking.
- Transportation request - Students who cannot utilize the campus bus services should fill out this form to be approved for additional van services.
- Interpreting / ASL request - Students approved for CART services or ASL should fill out this form to request services in their classes or other university-sponsored events and activities.
- Table and Chair request - Students approved for table and chair accommodations for a course should fill out this form.
- ODS Release Form- For release of student information to a third party under the Family Educational Rights and Privacy Act (FERPA).
Forms for Students Not Registered with ODS
- Inquiry form - Students or parents looking for more information about our office should fill out this form.
- Registration form - Student looking to register with our office to receive accommodations should fill out this form.
- Additional parking request - Students who do not have a state issued permanent or temporary (Handicapped Parking) ID or placard and require additional parking should fill out this form to be approved for additional parking.
- Transportation request - Students who cannot utilize the campus bus services should fill out this form to be approved for additional van services.
- Housing request - Students requesting housing accommodations for their disability or health conditions should fill out this form.
